Tibialis Posterior Dysfunction

Tibialis Posterior Dysfunction (TPD) is a common condition causing ankle pain and heel pain, found commonly in runners and athletes. It occurs when the posterior tibial tendon (a thin structure in your ankle) becomes inflamed or torn.

Symptoms of TPD could be sharp pains in the arch of your foot, a stiff ankle, a 'popping' sensation in your foot and visible inflammation along your tendon.

Stability and support along your foot causes heel, arch and ankle pain, and can lead to fallen arches or flat feet.

The Tibialis Posterior tendon arises from the back of your calf and travels down the inside of your leg, round the ankle and inserts into the bone at the top of your arch.

In classic Tibialis Posterior Dysfunction you will feel pain along the track of your tendon, usually from the foot up. It is worse on standing or walking. Some people report their ankles feeling 'weak', or their arches 'dropping'.

Don't try and run through this injury, or wait for it to get better - it's not worth risking the permanent damage to your foot. Take time off your feet, and if you must exercise focus on sports like swimming or cycling, as long as they're not painful.

Early intervention for TPD with an insole is essential - if not addressed then this condition has the tendency to become worse over time, even leading to a collapsed arch.

Contrary to popular belief, a custom-made orthotic doesn't alter and treat TPD any better than ready-made ones. The need to start treatment ASAP means that the X-Line TPD insole should be your first line of defence.

In addition to your insoles, stretching your calf is a good exercise for TPD. Use a therapy band or wobble board if you have one. Finally, ice packs can help control short-term busts of pain. For a more aggressive treatment, see a podiatrist to discuss the possibility of surgery.
